Discount Eligibility Request
Please complete the form in it’s entirety and then submit.
Once the financials are reviewed, you will receive a phone call to discuss the outcome and next steps. Submission of this form DOES NOT automatically register you for a Child Impact Class.
Please send an email to training@mhcgm.org with at least one of the following attached to be considered for a fee reduction:
- Copy of TANF Card or Approval Letter
- Copy of EBT Card or Approval Letter
- Copy of Medicaid Card or Approval Letter
- Copy of Food Stamps Card or Approval Letter
- Copy of Disability Approval Letter
- Most recent official letter stating your benefits
- Completed State of NH Judicial Branch Financial Affidavit (click here for blank form and only fill out page 1)
Your information will be reviewed against income guidelines to determine if you qualify for assistance. After a review is conducted, you will receive an email from training@mhcgm.org advising you of the findings, with instructions as to how to register for the course with any application tuition assistance.
