New appointment scheduling procedure to take effect at CSS

Effective July 1, Community Support Services moves to an Open Access model

As of July 1, 2024, the Community Support Services (CSS) division of MHCGM will transition to a new Open Access model for scheduling appointments. Community Support Services is a program for adults experiencing serious and persistent forms of mental illness, including co-occurring substance misuse.

The goal of the Open Access model is to get a patient into an appointment slot within 30 days of initial contact with MHCGM. Therefore, we will no longer schedule intake appointments for the vast majority of our CSS patients.

Any new or returning patient should call our main line at 603-668-4111, option #4, and speak with the scheduling department.  Once demographic and insurance information has been updated, patients will be directed to Open Access. Please see below for the new Open Access procedure for scheduling an appointment at Community Support Services.

Starting July 1st, Open Access will occur at 1555 Elm Street. Due to a move, Starting July 8th, Open Access will occur at 2 Wall Street, 3rd floor

Monday, Tuesday, Wednesday and Thursday

8:00 AM-12:00 PM

 

Open Access Procedures:

Step One – Phone Call: If an appointment is needed,patients must contact our scheduling department at 603-668-4111, option #4, to update their demographic and insurance information.

Step Two – Updating Information: Once step one is completed, the Scheduling Department will direct the patient to Open Access. Please note: A patient cannot arrive for open access until they speak with scheduling.

Step Three – Open Access Arrival: The patient arrives at Open Access and will check in with the support staff and complete preliminary paperwork. If the patient is not insured, they will be directed to meet with the Benefits staff to start the process of applying for insurance. After meeting with Benefits, the patient will meet with a medical assistant to obtain vitals, go over the medical screening form, and sign releases of information.

Step Four – Intake: After all paperwork and medical screening are completed, the patient will meet with a clinician for the intake assessment. This meeting can take up to 2 hours. Once it is determined a patient is eligible for services, the patient will be scheduled for an appointment with a medical provider within 21 days. When possible, CSS staff will try to accommodate the patient for an intake appointment on the same day they arrive for Open Access, but that is not guaranteed. In some cases, a patient may be asked to come in for the next open access day.

Parking at 2 Wall Street: There are a limited number of parking spots in front of the main entrance and directly behind the 2 Wall Street location. MHCGM also accommodates additional parking in our lower lot. The entrance is on Spring Street, and you will see signs that designate the lot as 2 Wall St. Parking.
