You can file a discrimination grievance either in person, by phone (603-668-4111 x4108), or via our website at mhcgm.org. If you need help filing a grievance, our 1557 Coordinator is available to help you.
If you have concerns regarding your care not related to discrimination, you may complete a complaint form at any of our locations or contact the MHCGM Complaint Administrator at (603) 668-4111 x4135.

Discrimination grievances with The Mental Health Center of Greater Manchester can be reported in one of three ways:

- By completing an online grievance form. If choosing this method, please click the link below and complete all sections before submitting. Once the form is sent, a notification will go to MHCGM’s 1557 coordinator who will follow up on the stated concern.
