Calling all InSHAPE programs, Health Mentors, Community Partners and participants from across the country! Join us for our 2nd Annual InSHAPE celebration and awards event! There is a virtual option via Zoom as well as an in-person option at 2 Wall Street in Manchester, NH! Don’t forget to submit your award nominations for the National InSHAPE Community Champion Award, the National InSHAPE Health Mentor Award, and the National InSHAPE Program Innovation Award! Send submissions NO LATER than September 13th, 2024 to thomasda@mhcgm.org! Please indicate in the registration as to whether you will be joining us in person or virtually! Light refreshments will be served.
